The government has set that as of September 2025, the amount of BPJS Health contributions for all classes remains unchanged and still refers to Presidential Regulation (Perpres) Number 64 of 2020.
Even so, adjustment plans are still in place, where the BPJS Class 1, 2, and 3 system will gradually be replaced with the Standard Inpatient Class (KRIS) by June 30, 2025, while the new contribution rates are still under evaluation.
The government is also preparing a contribution increase policy, which is still being discussed and remains a current discourse. However, this can be anticipated through strategic long-term household financial management.
So, how much are the BPJS Health contributions for 2025 across all classes? Check the complete information below!
Latest BPJS Health Regulation
On May 8, 2024, the government ratified Presidential Regulation (Perpres) Number 59 of 2024 as the third amendment to Perpres Number 82 of 2018 concerning Health Insurance.
This new regulation establishes the implementation of the Standard Inpatient Class (KRIS) as the minimum standard for inpatient services, covering hospital room facilities, number of beds, as well as supporting infrastructure.
KRIS replaces the Class 1, 2, and 3 system to provide equal service for all BPJS Health participants and to maintain equal access to healthcare in Indonesia.
The implementation will be carried out gradually until June 30, 2025, at the latest. However, technically it is not yet effective until new contribution rates are determined. The KRIS inpatient room criteria include:
-
Building components with low porosity.
-
Adequate lighting.
-
In-room bathroom meeting accessibility standards.
-
Proper ventilation and room temperature.
-
Oxygen outlet.
-
Complete bed facilities.
-
Separation of rooms based on gender and patient condition.
-
Bedside cabinet per bed, curtains between beds.
-
Room density meeting standards.
This policy does not apply to infant/perinatology wards, psychiatric patients, special facility rooms, or intensive care.
BPJS Health Contribution Amounts 2025
The BPJS Health contribution rates for 2025 vary based on participant categories. Independent participants (PBPU), wage-receiving employees (PPU), and contribution assistance recipients (PBI) each have their own rules as set by regulations. Here are the details:
Independent Participants (PBPU)
Independent participants (PBPU) are those who work on their own, such as entrepreneurs, freelancers, or other professions.
They can choose the care class according to their needs and financial capacity. The contribution rates are:
-
Class I: Rp150,000 per person per month.
-
Class II: Rp100,000 per person per month.
-
Class III: Rp42,000 per person per month (with Rp35,000 paid by participants and Rp7,000 subsidized by the government).
Meanwhile, BPJS Health ceilings differ by class. For example, the ceiling for normal childbirth is Rp600,000 in Class I. The ceiling for family planning services, such as IUD insertion, is Rp100,000.
Wage-Receiving Employees (PPU)
Wage-receiving employees (PPU) include government and private employees, such as civil servants, TNI/Polri members, state-owned/private company employees, and others.
The contribution is 5% of monthly salary, with 4% paid by the employer and 1% by the employee. This calculation applies up to a maximum salary of Rp12 million per month.
For additional family members, such as the fourth child and beyond, parents, or in-laws, contributions are set at 1% of salary per person per month and are fully borne by the PPU participant.
Contribution Assistance Recipients (PBI)
Contribution Assistance Recipients (PBI) are participants from underprivileged groups registered as recipients of government social assistance.
The contribution is Rp42,000 per person per month, fully covered by the government through the national or regional budget (APBN/APBD).
PBI participants receive the same healthcare services as other participants, with care rights in Class III.
Late Payment Penalties
BPJS Health late payment penalties apply to participants who fall behind, especially when they need inpatient care.
Based on Perpres Number 64 of 2020, the penalty is set at 5% of the initial inpatient care cost multiplied by the number of months overdue, capped at 12 months and a maximum of Rp30,000,000.
For PPU participants, the employer bears the late payment penalty. This policy serves as encouragement for participants to fulfill their obligations on time to ensure the sustainability of the JKN program.
Tips to Avoid Missing BPJS Health Payments
Paying BPJS Health contributions on time is important so that healthcare services can be accessed without disruption.
